To all Retired Officers (and Dependents): All of you have no doubt received the COH Dependent Eligibility Verification Statement in the mail whereby the COH is asking for your input to confirm any existing dependents on your medical plan and to verify their personal information. This information on the form includes NAMES, BIRTHDATES, SOCIAL SECURITY NUMBERS, ETC. and I for one find it very reckless that this information is being sent out in regular mail and in this manner. As a two-time victim of Identity Theft, you should be outraged that the City is doing this. Trust me, there are other avenues available to solicit information from us to include logging onto secure websites, etc. If social security numbers had to be listed on their forms, then why not list only the LAST 4 DIGITS of the number?
